Understanding Hallux Rigidus: Causes, Symptoms, and Treatment Options

Jul 10, 2024

Hallux rigidus—a term that may sound daunting but essentially translates to wear and tear. This condition, often stemming from prolonged and repetitive trauma, affects the great toe joint, resulting in osteoarthritis (OA) and degenerative joint disease (DJD). At Advanced Foot & Specialists of Arizona, we're here to shed light on this condition and guide you through its implications and treatment options.

Hallux rigidus involves the first metatarsal-phalangeal joint, leading to the deterioration of articular joint cartilage, thinning, and potential complete loss. Additionally, bone spurs may develop at the head of the first metatarsal and base of the proximal phalanx, exacerbating symptoms.

Degenerative joint disease affects approximately 30 million people in the United States, causing classic symptoms such as pain and reduced motion in hallux rigidus cases. This debilitating condition progressively degenerates both articular cartilage and subchondral osseous tissues, limiting functional capabilities.

If you're experiencing pain in the great toe joint, seeking guidance from a podiatrist is paramount. A comprehensive clinical evaluation, including x-rays, ultrasound imaging,  and a history and physical examination, will aid in devising an appropriate treatment plan.

Treatment options for hallux rigidus range from conservative measures to surgical interventions, depending on the severity of the condition. Procedures such as Cheilectomy, osteotomies, joint replacement, or joint fusion may be recommended by your foot and ankle surgeon.

For early stages of hallux rigidus in the great toe joint, joint salvage is recommended. While procedures like Cheilectomies and bone shortening osteotomies can provide relief, they may exacerbate symptoms if cartilage injury is present. Total joint replacement and joint fusion are better options for more severe degeneration of the joint.
